Bodywork Policies
Health History and Consent to Care
Prior to their first visit, all clients are required to complete a new client packet in full—including client information, health history, consent to care and liability release—and agree to adhere to Iron Bird policies, prior to receiving treatment or consult. Please arrive to your first session with your completed paperwork.
Clients are required to keep their practitioner apprised of any updates or changes to their health history and client information, including address changes, new health concerns and emergency contact person, and may be asked to complete the new client packet on a yearly basis.
COVID-19 Pre- and Post-Screening Procedures
With current COVID-19 concerns, all clients will be emailed a link to a screening form 48 hours prior to receiving treatment at Iron Bird. A response at least 24 hours before the session time is required to confirm your appointment. Failure to comply with pre-screening will require that the session be canceled and the client will be responsible for the missed session fee.
